    Funding & Eligibility

    How the Lumin CareHub is funded under Support at Home

    The Lumin CareHub and its services can be claimed across multiple Support at Home service categories - meaning different components of what Lumin delivers are funded through different pathways. A single participant may have Lumin funded across multiple categories simultaneously. Your team works with each participant's prescriber and care partner to identify the right funding mix based on their support plan and assessed needs.

    SEPARATE FUNDING POOL

    Not from your service-hour budget.

    AT-HM is a separate funding pool, allocated to each participant on top of their quarterly ongoing-services budget, and can only be spent on assistive technology and home modifications. The Department is explicit: quarterly funds cannot pay for AT-HM items, even when unspent.

    How providers claim the Lumin CareHub under AT-HM

    Confirm the participant's AT-HM balance

    Check My Aged Care for the participant's assistive technology tier (low, medium or high). This pool sits on top of the quarterly ongoing-services budget and cannot be substituted with unspent quarterly funds.

    Identify the assessed need

    Document the functional need in the support plan - social isolation, emergency response, falls risk, cognitive change, or difficulty using a smartphone. The need drives the prescription, not the product.

    Arrange the prescription

    An occupational therapist or other qualified health professional prescribes the Lumin CareHub and any peripherals. Prescription and wrap-around services are claimable as clinical supports with zero participant contribution.

    Map the item to the AT-HM list

    Reference the relevant ISO 9999 categories - personal emergency alarm systems (22 29 06), multifunctional communication systems (22 24 04) and signalling and localisation devices (22 29 03). Use the exact codes in the prescription.

    Request a quote from Lumin

    We supply an itemised quote covering the device, peripherals, installation, onboarding and ongoing managed service, formatted for AT-HM submission. Turnaround is typically two business days.

    Submit the AT-HM request

    Lodge the prescription, quote and supporting evidence through Services Australia. Where the request exceeds the participant's tier, attach the additional evidence pack outlined in the guide.

    Install, onboard and record delivery

    Lumin installs and configures the device, onboards the participant in person, and issues delivery documentation for your records. Onboarding and digital-skills support may be claimed under independence.

    Claim ongoing services correctly

    Telehealth, nursing and allied health delivered through the Lumin CareHub are claimed against the quarterly budget as clinical supports - never against AT-HM. Keep the two claim streams separate in your finance system.
